DISC Basic - 30 Minute Assessment Debrief

Your purchase includes access to the selected John Maxwell assessment and, when stated in the product description, one 30-minute debrief with Nequia.

POST-PURCHASE INTAKE FORM

After completing your purchase, you must submit the Client Assessment and Debrief Intake Form. The form collects your contact information and availability for a debrief session.

Debriefs are generally scheduled Tuesday through Thursday after 6:00 PM. The availability you submit represents your preferred dates and times but does not guarantee an appointment.

ASSESSMENT ACCESS AND SCHEDULING

After receiving your intake form, Nequia will review your availability and send you:

• Your John Maxwell assessment link or access code
• Your confirmed debrief date and time
• The deadline for completing your assessment
• Any additional preparation instructions

Your debrief appointment is not confirmed until you receive written confirmation from Nequia by email or Facebook Messenger.

ASSESSMENT COMPLETION

You must complete your assessment by the deadline included in your confirmation message. This allows Nequia sufficient time to receive, review, and prepare your results before the scheduled debrief.

Failure to complete the assessment by the deadline may result in your debrief being rescheduled.

ASSESSMENT RESULTS

Completed assessment results are delivered directly to Nequia for review and debrief preparation. By purchasing and completing the assessment, you consent to Nequia receiving, reviewing, and securely maintaining your results as part of your client profile.

ASSESSMENT ACCESS

Assessment links and access codes are assigned to the individual named on the order. They may not be transferred, shared, duplicated, or resold.

CANCELLATIONS AND RESCHEDULING

Clients must provide at least [24 or 48] hours’ notice to request a change to a confirmed debrief appointment. All changes are subject to availability.

Missed appointments, late cancellations, or failure to complete the assessment by the stated deadline may result in forfeiture of the debrief or a rescheduling fee of [$ amount].

If Nequia must reschedule your appointment, you will be offered another available time without an additional charge.

REFUND POLICY

Because assessment links and access codes are digital products that cannot be returned, purchases become nonrefundable once the assessment link or access code has been issued.

If your assessment link or code has not yet been issued, a refund request may be considered within [number] days of purchase. Payment-processing fees may be nonrefundable.
